The following additional materials must be received before 5 p.m. on May 12, 2023:
- A copy of your 4-year high school transcript and/or a college transcript to:
- Print, complete, and submit this certification form.
- Submit two (2) letters of recommendations from individuals with a direct knowledge of your abilities, accomplishments and potential. One (1) recommendation must be from a teacher or other personnel working at your current school. Those providing recommendations can submit them online or mail them in separately with this form.
- Provide documentation of any other scholarships/financial aid received to date (if applicable).
