Please read these instructions carefully before you begin:

Take care to submit well-written answers to the essay questions, as your application will be read by the selection committee. Write essay answers that fit within the listed word limits – these answers cannot be shorter or longer than what is required.

If you would like to prepare your answers to the questions in another document and copy and paste them into the online application once you are finished, you can find a list of the essay questions in this document, which you may use for preparation of the essays only.

You will be able to save your progress and come back to it later, but your link to return will expire after 30 days.

If preferred, a fillable PDF version of the application can be found here. We prefer that you complete this document on a computer, but it can be completed by hand if necessary. If submitting electronically, please submit in PDF format.

The APPLICATION must be fully completed by the applicant and received by Friday, April 30, 2021 at 5 p.m.
In addition to the completed application, all of the following materials must be received by Friday, May 7, 2021 at 5 p.m.:
  • A copy of your four-year high school transcript and/or a college transcript.
  • Two (2) letters of recommendation from individuals with direct knowledge of your abilities, accomplishments, and potential. One recommendation must be from a teacher, or other school personnel. Those providing recommendations can submit them online or email (preferred) or mail them in separately with this form.
  • Using our certification form, your homeless or formerly homeless status must be verified on a document signed by your school’s homeless liaison or a service/shelter provider.
  • Documentation of any other scholarships/financial aid received to date (if applicable).
